APPLICANT REQUIREMENTS AND INSTRUCTIONS:

Qualifications:

• Must live in Lauderdale, Colbert or Franklin county, Alabama.
• Must receive either Food Stamps or Medicaid and provide proof that you receive one of these.  Provide proof either by attaching below, mailing to 2701 Mall Drive, PMB 266, Florence, AL, or emailing to nasana2015@yahoo.com.
• Must complete an application (1 for each pet).  These can be completed using the form below, sent through e-mail, or hard copy via mail)

Copays: Must pay a copay of $25 per animal if the animal is up to date on rabies vaccination, $35 if the animal requires a rabies vaccination.. 

               This can be provided by:

•  Mailing a money order to the above address. If you have more than one animal, please submit only one money order for all.
• Using PayPal with the DONATE button on this website.
• Using a credit card or PayPal on our Facebook page with the DONATE button. 

Approval:      PLEASE DO NOT SEND COPAY UNTIL YOU HEAR FROM US!!!!

• After applying, you will receive a confirmation email. 
• Once we receive your proof of Food Stamps or Medicaid and Copay, we will contact you and schedule your pet's spay/neuter appointment. 
• Please check your spam email if you do not receive a confirmation email within 48 hours after you submit your application.

Owner is responsible for $10 of the cost of the rabies vaccination if the pet is not up to date, as well as any other costs deemed necessary by the vet at the time of surgery. 

*Limit is 5 animals per year, per household. Please submit separate applications for each animal, co-pays can be combined as one payment.
